Hospital Palliative Care Team (HPCT)

The team is led by the medical team, consisting of two Specialist Palliative Care Consultants and Staff Physician.
There are two Hospital Specialist Palliative Care Nurses who are based on the ground floor of Forest Holme. They visit patients within Poole and Alderney Hospitals.
The team can help with:
- Management of symptoms that are difficult to control
- Fears and anxiety about your illness and the future
- Advice on social and financial problems
- Help with applying for relevant benefits and grants
- Referral to Day Care
- Dietary advice
- Liaising with other healthcare professionals between appointments e.g. Hospital Consultants, your GP or Community Nurse
- Support through palliative chemotherapy/radiotherapy
- Spiritual needs
- Advice about other services that may be available to you
- Encouraging and enabling you to maintain your independence
The team is available from Monday to Friday 8.30am to 4.30pm.
Community Specialist Palliative Care Team
The team is led by the medical team, consisting of the two Specialist Palliative Care Consultants and Staff Physician.
There are seven Community Specialist Palliative Care Nurses who are based on the ground floor of Forest Holme. They are linked to all of the GP surgeries in Poole, Wimborne, Cranborne, Sixpenny Handley and the Isle of Purbeck.
The team work within the community, but provide the same range of care and advice as outlined above.
The team is available from Monday to Friday 9am to 5pm. Outside these hours the ward staff at Forest Holme can be contacted. They are able to offer telephone advice and are supported by the medical team 24hrs a day, 7 days a week. This advice is available to patients, carers and health care professionals.
